The answer is always 13. If you get it wrong on your first attempt, it gives you the answer and another chance. The challenge itself keeps the spammers out, and the difficulty of the questions keeps the drive-by trolls out.
It may be strange, and it may lower the interactivity of my blog, but it also makes administration easier, and I'm all about easy.
If so, I highly recommend http://wordpress.org/extend/plugins/yawasp/ -- there's no captcha, no user internaction (it looks and feels exactly the same to users), but it does some behind-the-scenes stuff that so far has been 100% effective at blocking spammers.
Akismet is also handy, and I think they make plugins for all sorts of blogging software for it.
Recaptcha.net is probably the best captcha service around, particularly because you help in "reading" books.
